The British Red Cross is working with the NHS to help people leave the hospital safely, and at the right time. The assisted discharge service ensures people are not unnecessarily admitted to, or delayed in leaving hospital to the detriment of their well-being and independence. This service helps assist people home from hospital, whether they were inpatients or attended A&E. They can provide support for up to 72 hrs, to help settle the client back in and ensure their safety. The service seeks to improve patient flow, reduce delayed transfers of care and avoid unnecessary re-admissions.
