Simply Connect North East Lincolnshire is a partnership project for the community, delivered by NEL VCSE Alliance and North East Lincolnshire Council and Clinical Commissioning Group.

We’re on a mission to help you find local community services, activities and support that can help you whether you are struggling with an issue or looking for new opportunities and experiences.

Maybe you’re looking to meet new people – or get advice or support for a friend or family member? Or you might be wanting to find out about exercise or social clubs, or how to get involved in a local charity?

Community services can provide specialist support too on a range of topics, such as domestic abuse, helping to manage your finances, housing issues or bereavement support.

We can support you to improve your health and wellbeing by:

Would you like some 1-1 support to decide what services may be available to you?

The local NE Lincolnshire Simply Connect team can help you to find out what issues you are struggling with and refer you into local support services that can help you.

This is known as a social prescribing service and is something that our partners have recently funded as an extension of the services provided by your local GP.

New Link Workers have been introduced to assist GPs in caring for a person’s health and wellbeing.

These workers (along with other health and social care professionals) can help you access services that can really help you with whatever issue you are experiencing.

This could be specialist support such as managing your health condition, bereavement support as well as keeping active and socially connected.

What is social prescribing?

This is when you are referred or signposted into non-clinical services by a professional as a way of supporting or improving your health and wellbeing.

Social Prescribing Link Workers are new posts, funded by the NHS, who will support you and make referrals on your behalf into community services.
