District Nurses - CPG

Patients must be registered with a GP in North east Lincolnshire. Initial access to support from the community nursing team is through GP referral. 

The district nursing team provide a range of interventions including:
- Assessment of health and social care needs, formulating an agreed care plan
- Chronic and acute wound care
- Continence assessment and advice
- Education in the prevention of pressure ulcers and provision of equipment
- End of life care
- Support and advice for patients and carers in self-management of their care
- Venepuncture for management of patient's long term conditions
