Social prescribing is a way for local agencies including GPs and other health care professionals to refer people to a link worker. 

Link workers give people time, focusing on 'what matters to me' and taking a holistic approach to people's health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.

Who we support

The programme currently supports people between 18 and 75 with the following long term health conditions:

• Asthma
  

• COPD
  

• Diabetes Type 1 & 2
  

• Hypertension
  

• Atrial Fibrillation
  

• Epilepsy
  

• Fibromyalgia
  

• Osteoarthritis
  

• Osteoporosis and
  

• Chronic Heart Disease
  

Support is offered for a 2 year period. Patients may also self-refer into the programme.
